It is our priority to keep our consumers educated and aware about their rights and responsibilities. These documents are for your review and reference, if you would like a printed copy, or if you’d like to complete any of the forms listed, please feel free to discuss this during your appointment and we will mail a copy to you.
Welcome Packet: This informational packet provides guidance on what to expect as our valued consumer. It contains a letter from our CEO, consumer rights and responsibilities, and consent forms. Please note that your assigned clinician will brief you on the consent process and will ask for you to verbally indicate your consent for Lifeworks NW to offer you treatment through both Telehealth and electronic communication.
Self-Pay Fees: The Fee Agreement helps you share insurance information and lets us know who is responsible for paying fees not covered by insurance. Our full fees are listed, as well as a grid to determine if you qualify for reduced fees on a sliding scale. For our King, Rockwood, International Way, Beaverton and Hillsboro Walnut sites 18JAV will not discriminate in providing services to an individual based on the individual’s inability to pay, whether payment for those services would be made under Medicare, Medicaid or CHIP.
Telehealth Guide: To reduce the spread of COVID19, and ensure the safety and health of our consumers and staff, we are now offering our routine services by video or telephone. We are using the Zoom meeting platform for telehealth sessions. For meeting information, appointment reminders can be sent via text and/or email with all details needed to participate in our sessions. Release of Information Form: This form gives 18JAV permission to release and receive your protected health information (PHI). While our responsibility to ensure your safety and the safety of others sometimes limits confidentiality, we make every effort to follow the consent outlined in your release.
Formal Complaint Form: It is our mission to positively support our community. If you have any concerns regarding services at 18JAV, please review our complaint packet for our grievance procedures. If you would like a paper copy, please reach out to our Integrity Line at 503-645-3581, extension 2450. Please leave a message, and we will respond within 10 business days.
